Use this form to report compliance concerns, fraud, waste, abuse, privacy issues, agent conduct concerns, marketing violations, enrollment concerns, whistleblower matters, retaliation concerns, or supporting documentation related to Medicare or insurance activity.

What to Include

Please include the date, people involved, consumer name if applicable, carrier or plan name, summary of concern, screenshots, emails, call recordings, text messages, mailers, applications, forms, or any other supporting information.
