By submitting this CVS Campout Reservation Request, I understand and agree that, once approved, these events are filed and treated as sales events under CMS guidelines.

I acknowledge and agree to the following:

• Approved CVS Campout events are considered Medicare sales events and must follow all CMS Medicare marketing and communication guidelines.
• I am responsible for conducting the event in a compliant and professional manner.
• I will not market, promote, or discuss non-approved products, carriers, or plan information outside CMS guidelines.
• I understand that any approved event must be canceled at least 14 days prior to the scheduled event date, when cancellation is necessary.
• I will notify Advantage Plus immediately if I am unable to attend, need to cancel, or need to make changes to an approved event.
• I understand that failure to follow CMS guidelines, carrier requirements, or Advantage Plus instructions may result in cancellation of my event access or future reservation opportunities.
• I understand that all information submitted must be accurate and complete.
