Carrier name and product line, such as Medicare Advantage, PDP, Med Supp, Life, Ancillary, ACA, or Group
Issue category, such as contracting, certification, RTS, portal, enrollment, commissions, provider/network, or other
Writing number, agent ID, or carrier portal username if applicable
Client/member name, date of birth, application confirmation number, or member ID if the issue involves a case
Clear description of what happened, what should have happened, and what support you are requesting
Screenshots, carrier emails, statements, application confirmations, error messages, invoices, or other supporting documents
