Scope & accuracy: Statewide listings below use CMS 2026 and 2027 landscape records for individual Medicare Advantage, MA-PD, and Special Needs plans in California. Landscape records provide plan availability, premiums, medical MOOP, Part D deductible/threshold, and star fields; they do not provide every supplemental benefit amount. Detailed 2026 service cost shares and optional benefit packages are from the CMS MPF Benefit Summary file. Detailed 2027 dental, vision, hearing, OTC, and extra benefit amounts are shown where the plan Summary of Benefits was supplied or Plan Finder data was personally checked. An unverified or blank field means “check the linked 2027 plan document,” not $0 or not covered. Plan Finder availability depends on ZIP code and eligibility.

Use this in an AEP review

Confirm the exact plan and member situation before comparing extra benefits.

1. Match the product

Check the contract and plan ID, full plan name, county/ZIP, SNP type, and eligibility. Plans with similar names may have different PBPs or service areas.

2. Separate medical and drug costs

Medical MOOP excludes Part D prescription costs. Compare Part C premium, Part D deductible, drug tiers, pharmacy network, and drug-specific estimated cost separately.

3. Confirm benefits in Medicare.gov

Open Plan Finder for the member’s ZIP and eligibility, then confirm Summary of Benefits, Evidence of Coverage, providers, prescriptions, authorizations, and limits.

Sources, methodology & limits

Plan-year fields come from CMS published downloads and Medicare.gov Plan Finder.

2027 and 2026 plan inventory: CMS annual California landscape files list approved MA, MA-PD, and SNP contracts, plan IDs, service counties, premium, MOOP, Part D deductible and out-of-pocket threshold fields, and star ratings. Exact PBP/contract/segment IDs are used for matching; additions and removals can also reflect changed identifiers.

2026 details: CMS CY2026 Medicare Plan Finder Benefit Summary and Optional Supplemental Benefit Summary files list plan service categories, cost sharing, flags, and packages. Benefit rows are shown under each 2026 record.

2027 spot checks: Medicare.gov Plan Finder was reviewed on October 1, 2026 for ZIP 90012 in Los Angeles County. It returned 70 non-SNP Medicare Advantage plans; ten first-page plan details were checked for premium, MOOP, primary/specialist copays, and service-level dental, vision, hearing, OTC, transportation, fitness, and other benefits. Those Plan Finder observations are tagged as Los Angeles ZIP 90012 data. SCAN and Humana 2027 plan-level highlights are also taken from supplied Summary of Benefits materials.

Changes: Red/green change badges are calculated only from matching CMS PBP identifiers. Lower MOOP is generally a lower exposure limit for covered in-network medical services; it does not indicate better overall value. Any plan crosswalk or changed identifier requires manual confirmation.
